This restructures the xml-metadata module to make it _actually_ work

acoburn requested to merge rework_xml_transform into master

At least it works for OAI_DC export endpoint (MODS hasn't been touched). It adds integration tests, a valid XSLT for DC and support for an OPTIONS endpoint.
